When i was there for a conference in February 2017, they used to have a shuttle from the hotel to UMN campus which would also pass through UM Hospital.
Yes they have a shuttle there. The only place they don't shuttle to which is very bizarre is the Metrodome. Not sure why but found this out while visiting. The train does however.

Your going to pay for parking no matter what. Just park at the main clinic valet and it'll cost you 6$ a day if you go to the correct building. The other one will cost you closer to 15
Yes, they have shuttle rides to and from the U Hospital and their clinic locations. Just have to give the front desk a heads up on when you need to be there.
