You must call the social and they will give you an appointment by cell or require you to appear at an appointment that they will give you to present documents. Many times they ask for children and family documents with the start date of medicaid...which will cover the medicare premiums...you must call the field office to which you belong...I do not recommend filling out the application on the page because it is extensive and sometimes difficult for certain people
