Ok, I've lost my license MANY times in the past few yrs. primarily due to unbridled irresponsibility. Everytime. I'm issued a replacement, I have to have a certified copy of a birth certificate issued by the department of health, AND a piece of mail with your name and current address on it. OR, TWO pieces of mail with your name and current address on it.OR if it is just a copy of what you already have that you need, you can probably either use that or just slap it down on top of machine and press one.
