NC. I need to get a replacement card for my 16 year old. I have his birth certificate, Xerox copy of missing s card, a drivers ed eligibility certificate with his name and an unofficial school transcript with his name, and d.o.b. is this enough

I work there and for minors to obtain a replacement cam use certified school records in a sealed envelope, medicaid card, passport, Insurance policy. Or medical record signed or stamp from a physician Birth certificates are not accepted to prove identity. It only proves age. No copies of the above is acceptable.

Most likely not. The transcript must be sealed originals from the school . Photocopies will not work for anything. The drivers Ed paper won't work. Drivers license, Medicaid card, passport, school I'd with photo, sealed medical records, sealed school transcripts are about it
