Go to front desk with spouse and ask to ad on a spouse, and they will take picture and create a card right there.
If your asking you probably are not the primary card holder she will need to get her own
Go to the member services desk.
You and your spouse go to the customer service desk and request one, your spouse needs to be with you, they will need to take a picture.
Go to the Service desk
Ask the Service Desk.
Have the person with the card add you
