How do I get my medical records and results of CT Scan and blood work sent to my family doctor,

Hi I work at advent wesley chapel. If you go into the ER there is a window to the left before you reach the check in desk you let them know which records you are wanting and they will print or put on a disc what you need after you sign a release for then you are free to give your records to whom ever you want.

I would think you would need to sign a release waiver to have all your tecords to your doctor. If you are referring to Moffitt records I would call their main number fir instructions on how you could have them sent
