Get your paperwork and turn it in to the front desk were your Dr was seeing you. Make sure you get the person name you gave it too, front n back office. They will give it to the Dr n it's takes the 7 to 10 working days. Find out when you can pick them up or call before you go.
Bring in paperwork personally. Have docs nurse come out to see you and give it to her/him
