Do they have forms or something to fill out if it's your first time donating plasma just so they know if your healthy enough for donating and if they do have forms to fill out do u have to fill it out everytime you go in to donate.

Yes, you'll have some basic paperwork, proof of address and id. Then you'll watch a video, you'll be given a basic physical, and walked through the process you'll be doing every future visit. You will not have to do this every time. You get a physical every 6 months. 1st visit takes an average of 2.5/3 hours, every future visit usually take an average of an hour to 90 minutes. More or less depending on the line, and time of day. I go between 10 and 11 am, and the wait time is minimal.
