Hey Vicki, You can go to SSA.GOV and search the info you can do it straight from the website. Is an article from experian.com and it will tell you what you need you may need to sign up for a "mySocialSecurity" account. https://www.experian.com/blogs/ask-experian/how-to-replace-a-social-security-card/
Hi. From experience. I called SSI in person to get my SSI card that was lost/ stolen. Call SSI, report need of what you asking for. Only you know your basic info about u, DOB, SSI #. Or can go in person on walk in time.
You can request it on line or walk in or call....all the above.
