If you do not have a gold star in the top right corner of your license, you will need to provide us with a certified birth certificate or a valid passport, your social security card and two proofs of your current Florida address. If you have had a name change, we will be required to see proof of that, the documents you provide must be the original copy.
For a US citizen Passport or birth certificate, SS card, and two proof of address, For a non US documents from immigration or green card and all the documents mentioned before.
