If you DO have a gold star, you will be required to bring in two proofs of your address- any kind of mail with your name and the new address. If you DO NOT have a gold star on the top right corner of your license, you will need to provide us with a certified birth certificate or a valid passport, your social security card and two proofs of your current Florida address. If you have had a name change, we will be required to see proof of that, the documents you provide must be the original copy. I hope this is helpful.
If you already have the star in the upper right hand corner you will only need 2 proofs of address. If not, you will need to bring a borth certificate or passport and your social security card.
