As far as trails go i would say it is easy to moderate difficulty. The trail to the location is prety eazy, however the last bit you need treck down to actually see the heart. Is NOT super easy. I would imagine it would be fairly difficult with a cane, but not impossible. It's the climbing back up to the main trail thats the hard part. I would use caution and make sure you can get out before going down. Just be aware its not quite as easy as the others have implyed.
Yes, the path is fairly level with a few moderate inclines. Just allow some extra time and enjoy.
