All the above is correct. But if its for a minor also have a valid CA ID with picture or valid school ID, Social Security card, birth cert. Also would be good if u have the minors entertainment work permit too if not once u get it you can email it to them.

Government picture ID like drivers licence and social security card or passport is most common. But there are also work visas. Always go to their website to see the actual registration process. It takes about three hours and they are very strict. Also bring dues paid up SAG-AFTRA card if you are union. They will check to see if it's paid. A receipt that it's paid from sag will work too.
