You cab either come in and get a visiting form or the offender can mail you one. Send it back signed, dated etc and their CCO will receive it and go through the process of approving or denying it.
Go in and get a visiting form, it's different from the prison one, and fill it out. Takes about 2 weeks give or take, to approve you.
