That permit is issued at Department of Licensing at 12801 E. Sprague Ave. 921 2358. Valley location. Call them for questions. Drive test is not offered there. I believe only written tdst.
No that's done at the DMV.... this place is for licensing your vehicles and getting tabs ect.
