Parts of the Cavern are, There are access ramps and modifications. Depending on your mobility and limitations some parts could be more challenging than others but absolutely they will accommodate as best they can.
I am disabled and can say that depending upon the nature of the disability, it may be possible to visit a portion of the cavern. That being said, it is dimly lit, has lots of steps, is winding with often wet walkways. I would recommend contacting management at Howe Caverns directly and speak with him regarding your exact concerns and needs. When I attempted to transverse the Caverns I was in a situation where it could not handle and had to leave. Best of luck on your venture.
Some parts of the cave is accessible. Some parts will not be.
I would recommend contacting Howe Caverns directly. They could tell you exactly how much of the cave would be handicap accessible. I forget where the first set of stairs is. It's not that far into the caves, unfortunately.

They do not have handicao access in the caves but have activties on land but wouldn't be too fun for an adult.
Having recently been there, i would say that it could potentially be done. However, the boat ride is not wheelchair accessible, and the cave does get very narrow at points. Additionally, there are sections of the cave that can only be accessed via stairs.
