Great question Tiff! We ask that you bring identification, copy of your insurance card, a list of your medications, allergies and questions you would like to discuss with the provider. If you are a new patient, please let us know who your previous provider was and we will assist in having your records sent to PCHS. If you do not have insurance, please bring verification of income and we can assist you with applying for insurance on the Washington Healthplanfinder or our sliding fee scale, whichever is appropriate. Do you want to be a permanent patient or just go for a single visit. They do keep you on record regardless though. So just your identification and a form of payment.
