Registered qualifying patients have the option to extend their registry card for one year or two years. Can I extend my registry identification card on-line? No. You must extend your registry identification card using the paper form which will be sent to you in the mail. No you have to renew your card through the state of Illinois. A letter should of been sent to you. If it was not please contact http://www.dph.illinois.gov/topics-services/prevention-wellness/medical-cannabis
Unfortunately no...you have to renew through the state. The certification is good from doctor for three years. An application for renewal should be simple. If you are a patient with PDI, Im sure some assistance from them could help. But the dispensaries are not where it is renewed.
