You now also need your social security card itself, not just reciting the number. I was told this and police officer clarified the rule change. You’ll need old license, green card if not a citizen, possibly proof of address. Not too sure in the last item.
It depends on the license you want. I would always suggest going to their website at: https://www.bmv.ohio.gov/dl-identity-documents.aspx. I know if you want a compliant license(one that you can use to get on domestic flights) you need (number of)document(s) to prove: -(1)your full legal name -(1)date of birth -(1)us citizenship -(1)social security number -(1?)name change "if your current name isn't on the documents" -(2)current address. So I am taking my Passport (full name, birthdate, and citizenship), a w2(social security number and current address), and an electric bill(for the second address need)
