I saw Dr Shane tucker Ent.. in April I having sinuse problems..my head and ears were stopped up and I couldn't blow out nothing..he gave me some antibiotics that didn't help then went bk for xray...and there was still infection..my head and ears still hurt...he did nothing to help me..and acted like he didn't care...so I hv to go see another dr...I wouldn't recommend this dr...

The medical records department is on second floor of the hospital. A picture ID is required and you'd sign a release of information form.
