You can walk in. They may or may not ask you to check in on the app and wait outside, though.
It depends on if the new phone is compatible with the Verizon Network. You will probably want to either call in or go to the store in person. There was no activation fee when I got my phone set up with Verizon. The plans vary in type and cost. The unlimited plans start at $70/mo. but are reduced by the number of lines you set up. It will just depend on your needs. Again, i would reccomend calling the store or visiting in person.
