I realize that this is an old question, but I'm going through and answering them anyway LOL. Unfortunately, on the S5 to get to the charge port you have to lift the screen. This is a high risk maneuver and the screen is more expensive than the device. We typically suggest getting an external universal battery charger, as it's more reliable and less expensive.

If the phone has been up to date with its updates, you'll probably have no way of unlocking it. If you remember setting a Samsung account on your phone, you may be in luck, try their find my Samsung website as they have a button to unlock the phone remotely. Otherwise your only method may be to erase everything on it and be sure you know the Google account you used on the device to be able to get back into the device since Factory Reset Protections exist.

There are ways to get it out. Depending on which version it is there are multiple version of the PS4 and they have different manual ways to eject the disk or you can bring it in and we can help you, OR if that doesn't work we can disassemble the entire machine, clean it out while we're in there, and remove the disk for $65
