You're going to have to have a photo ID or an up to date medical record with the doctor's signature on it. It can be an employers ID or school ID. I tried for months trying to get a SS card for my son because he lost his wallet with his SS card and ID in it. We were told that they no longer accept birth certificates as proof of identity. The SS Administration has made it very difficult in getting a replacement card.
