Thank you for your inquiry. Unfortunately our Apache Junction location does not offer vital records services. This service is only offered at our San Tan Valley, Casa Grande and Maricopa locations, however you may go online and mail in a request.

We also have an separate department that works closely with the state health department that provides us updates to ensure you are receiving the highest quality of care.

You may visit our San Tan Valley, Maricopa or Casa Grande location in person to obtain the certificate. You will need to bring proof of relationship, a valid government-issued photo ID, & they will also have you complete a death certificate application. Fees do apply.
