I was working here today 5/2/22 at 10:00am an some lady order me to stop speaking Spanish and when try to look at the tag with her name she said don’t need you to see who I am I need u to stop speaking English I felt offended and discriminated by whoever this lady was I called my boss who also back me well shortly after this lady call me to her like I was an animal and In her own words she said let me make myself clear when she uses that tone I ask for her to speak to me with respect the shortly after she said I don’t have to talk to you anyways because you gone turn around and leave so I told her I would report her for discrimination she said she don’t care who I report her too I clearly said ima report her and will be getting a lawyer this again the law she said I don’t care I run this place and you can leave now this is why this place is so short staff people come to works and take care patient not be discriminated abusive nor treated like an animal in my own words I was embarrassed humiliated and discriminated hope you know this person shouldn’t be running this place if you must know I’m Marydelly Montoya

Incapable of returning phone calls. Not a good sign. You know who you are. Don't act like your concerned now. Multiple calls were ignored, Lots of time has passed. Unless there is possible money coming there is no courtesy about returning calls.

No matter what language you speak, we always make sure our staff is able to communicate effectively with our patients and their families. We have multi-lingual individuals on staff, but if we do not have a staff member that speaks your specific language, we will provide a translator.
Insurance coverage is dependent on the specific policy, but the majority of our services are covered under most insurance plans. For more information, please consult our staff to discuss your personal insurance situation.
Out-of-pocket costs are dependent on insurance coverage. For patients covered by Medicare, 100% of the cost is covered for the first 20 days; and 80% for days 21-100. If the resident has secondary insurance, then this policy will pick up the additional cost not covered by Medicare. For patients without secondary coverage, the resident or responsible party will need to pay the remaining 20% for stays over 20 days. For non-Medicare health insurance plans, costs vary by plan. Please discuss your personal situation with our team.
The length of stay is dependent on a number of factors, which are different for each patient including clinical needs and progress made in therapy. Please consult our staff about your family member’s unique situation.
Family members, per resident’s request, are updated within 24-48 hours of admission along with a care conference call scheduled within 72 hours.
Yes, of course. Our team is happy to help your family connect over FaceTime. All that we ask is for family members or friends to call the front desk to schedule a FaceTime visit so that we can be available to assist.
The amount of therapy a patient receives will depend on a number of factors. Each patient has an individualized care plan with the ideal amount of therapy for their situation. Most patients receive up to 3 hours of therapy a day.
We are carefully following all guidelines outlined by the CDC and IDPH regarding COVID for patients, staff, and visitors. We also have a strict isolation unit for the treatment of COVID patients after their hospitalization, ensuring there is no contact between these patients and other patients in our facility.
Currently, due to COVID, we are not allowing visitors to stay at the facility. We are honoring CDC recommendations and allowing visitations for one or two family members if a loved one is in hospice. In addition to following CDC and IDPH guidelines for temperature checks, for masks, and additional PPE being worn at all times while in the room.
Normal visiting hours are 8am to 8pm. Currently, due to CDC guidelines and IDPH, we are offering face-to-face visits conducted with glass separation and a phone. This option is available if the patient can safely be out of their room.
