Call Lebanon VA
We have paperwork here at the clinic to fill out about getting him back in the system with us! Or, of course you can always go to the Lebanon VAMC and do the same - either way it goes to the same place! I would recommend doing LVAMC only because it is faster (they register him as you guys talk to the person at the desk vs us inter office mailing the application) It is up to you how you guys want to proceed :)
The audiology department is still located at the Lebanon VAMC. The Lancaster clinic has Primary Care, Mental Health, Optometry, Telehealth and soon to be Physical Therapy. The contact # for Audiology would be: 800-409-8771 ext 4582

Labs wise, yep (even though they have to be transported to Lebanon to actually be read) - imaging wise that would be done up at Lebanon. But if you have a provider here at Lancaster that would all be set up for you! All you have to do is ask :)
