FACE MASK REQUIRED!!! Once you enter the building the front desk has disposal masks. Great place and friendly service. The facility DOES NOT provide medication narcotics for pain. We waited about 1 hour to be seen. Plenty of parking. Doctor that attended my son was very nice and she also was very sweet. Comforted my son and myself.

Hi Nick, our cash pay DOT physical exam costs $89. We do accept some insurances for DOT physicals - please call our front desk at 847-901-8400 to inquire whether your specific insurance typically covers DOT physicals. Thanks.
You have to call the number on the door of the office as they are not allowing anyone to just walk in
Our self pay Tb test price is $34. With United it typically should be covered but we recommend asking your insurance carrier.

Hi Nea, We are in-network with most major commercial insurances, as well as Medicare and Tricare, but if not using insurance then we offer a self-pay office visit for $99 - this would include the consultation and a prescription to your pharmacy, if needed. Otherwise there is a fee for additional services such as flu/strep testing, xrays, braces/slings, etc. but our staff would talk to you about that cost before doing anything.
Hi Cinthya, it’s $45 for a 5-panel drug screen and $50 for a 10-panel drug screen. If your new employer doesn’t specify, then a 5-panel is likely sufficient.
