Five stars for Brenda(nurse) She’s professional, friendly and very caring!!! I went to the Heber Health Department since the Park City Health Department is a total mess, sadly.

I've contacted this particular health dept. location for (2) international trips to different areas in South East Asia. Both experiences were 5 star, the staff who I was put in touch with were all friendly and patient going above and beyond to assist with my questions and needs. They seem like they really care about your health and well being plus they offer great advice for first time travelers. If you're going on any big trip outside of the US where vaccinations may be required I highly recommend going in person to have a consultation.

Good day! Is there any chance you can mail my food handler ID card?? I took the exam on March 3,2017 and until now I didn’t receive mine yet. My brother who took the exam with a same day as mine already receive his card. Please & thank you!

I am so sorry, I did not see your question until just now. If your question hasn't already been resolved I am happy to help out. I checked our food handler database and could not find any record of you. Just in case you were listed somewhere else, I looked up your name in the database for the last couple years and only found five people named Alyssa (none spelled of Allyssa) - none of which matched your last name. To help me continue the search, can you tell me which company you used for your training? I can contact them to see what happened to your record. Thanks
