Had a heart attack early morning of June 3 and the fire department was at my house in no time and rushed me to the hospital for quick procedure to open up my closed valve. I’m only 49 but a heart attack can happen to anyone!

Typically no, emergency providers transport to the closest accepting facility in the presence of an emergency. We do make exceptions based upon the medical needs of the patient. However preference is not usually a consideration. We will take into consideration if a special needs patient is being transported, or a pregnancy emergency, as well as any high priority special call such as a trauma/burn/Neuro/ or other specialty facility transport. If you are having an emergency you should be transported to the closest accepting facility, if it can wait (meaning you can transport yourself, or your family can transport without need for immediate intervention) or your primary care physician can't/won't see you then you may transport yourself to the preferred facility. Hope this was helpful.
You can call them and they will come out to assist you.
We don't have a standard place to look for our citizens' MOLST forms. We used to have a program that placed stickers on the inside of the door jamb and a vial in the freezer with important information. Unfortunately, folks often struggled with maintaining their documents. We will ask caregivers for this information and we usually find the information co-located with a patient's medications.
