Isabella rehabilitation is a dam joke i am being nice by giving them a one star. My first night I felt unsafe because they didn’t have the correct equipment to assist and dinner was gross. The next morning at 10:30 I asked for assistance to the bathroom and no one came therefore I had to urinate on the floor and right now it is 11:50 and no one has come to put me on the toilet but someone did come clean the room, I also still don’t have a remote for the tv dam shame.

This is the worst place you can put your love ones. Staff members all have attitudes and will leave patients unattended throughout the day, which caused my mom to have bedsores and I try to call the social workers or anybody left numerous of messages they never return my calls. My mother was there when Covid hit and was stuck in there the whole time while she was there she lost 75 pounds within the time. I would bring her foods she likes they would never give it to her. They will let it rot on her side table. Then my mother fell on the floor allegedly at 10am I was not notified into 3 PM. They didn’t examine her to see if there was any internal bleeding or anything it took me to come from Brooklyn to Isabella for them to finally called the ambulance just so she can get checked and make sure she was fine. I removed her from that facility in January 2022 till this day, They are still collecting her Social Security checks which I’ve been fighting for this whole time. Nobody returned my calls. This place should be shut down.

There's actually a fantastic adult day care program! They provide breakfast and lunch and lots of fun activities! I know because I worked there.
There is a parking lot behind the buildings. The last time I was there it was free.
Parking is free and you check in at the building on 515 Audobon HOWEVER read my review. If it's for a loved one, please find another place!! Isabella's is deceiving visually but go to Dementia unit... Rampant and ignored Elderly abuse and neglect.

I believe there's at least one staff member who is a Notary. Call the Center and ask. Otherwise the UPS stores at 4768 Broadway and 809 W 181 St. say they have a Notary on site, call them.
