No, unless they have recently changed that. First you will have to pick up the forms and take them to your doctor to be filled out and signed. After that just take them back to the DMV and then if everything is okay on the forms, they will issue the placards then and there. That is all I had to do to get mine. Only problem is you might have to wait a little if they're busy, which is almost everyday, lol.

You can pickup the paper you have to fill out and just drop it in the drop box in an envelope with the $15 payment and a phone # and they'll call you when you can pick it up
