I'm a patient at another St. Thomas facility, so I'm not sure if this option is only for existing patients. But for my recent appointment at the Howell Allen Clinic, they sent me an email a few days ahead and it had a link where I could go into the Web portal and fill out the medical questionnaire. So on the day of the appointment I just had one paper waiver to sign and that was it. No orange pad or any other paper forms.
