You are supposed to put ballot in brown sleeve. The brown sleeve is also the instructions. In my opinion, the instructions should be separate. Do not forget to sign and date back of your envelope. Before placing stamp or hand carrying to supervisor of elections, sign and date the back of envelope.
Yes. You fill out the application then they will mail you the card.
Yes! There's a ballot box right inside the door.
