If there was a way to give this place no stars then I would have. This is the most unprofessional place I have ever seen. My sister has been disrespected so many times at this place. The care she has (not) received is not even reasonable and I wish there was actually someone who worked here that really cares about the people they are servicing. This place needs to be shut down immediately.

https://opendoorclinic.net/contact-us/ If you call, leave a message. They are staffed by volunteers and it may be a little while before they call you back. Also, if you plan on going in to pick up the application, note their office hours to avoid a wasted trip. Good luck!

They determine your eligibility by your income. You'll need to pick up an application and then they'll call you with an appointment to determine your eligibility. They are staffed by volunteers so make sure to check their website for their operating hours. It is not your typical Monday through Friday 9 to 5.
