After having to wait a year because of change after change. Then a canceled appointment a couple days ago it was great to have it go as well as it did today. Thank you all again

I have had my ID Card stolen I can't make an appointment because it appears no answers the phone this is an emergency situation in that I have several Doctor appointments coming up and they will not take any appointment without this card. I am coming from Wilmington,
2 forms of ID. Your current ID and your driver's license will suffice.
The sponsor needs to complete the paperwork (DD 1172) for dependent children between 14 to 21, to 23 if full time student. Hope that helps.

Lost form has to be taken not only to PMO, but to your supervisor as well - military active duty must see E-8 or above.
After 1600 you go to building 24 (MITSC-East) for after hours, weekends and holidays.
