Home health companies need to be monitored more and properly. My child was hurt due to neglectful conditions by an aide from one of the companies and nothing will be done so it doesn't happen to others. Shameful.

My daughter has autism, cva,mild retardation and ADHD and past three years a worker there by the name Carnell keeps on denying my child I be seending all the information she keeps like her school iep my work make 121 a week and my school info she says that iep from her school should be 20 pages I sent in what the school gave me before they went on summer break come on those who are fakeing to get personal care assistance is one who can get it but those who can't help themselves are the one who are turned down all autism children do have care need problems just like any one else's .

*Beware* This company is extremely unprofessional and does not follow through with authorizations for patients. I have been waiting for weeks for information on the status of a request for services. First the wrong form was sent from their office in which I had to contact them for the status. They claimed they would send the correct form and my doctor has yet to receive it. No one from Liberty ever contacted me to update me and each time the person that answered was rude. Now it's time for me to return to work and I have wasted all the time that help was needed asking them to simply follow through. If you want to receive care during recovery I suggest you to contact an agency that will be considerate of your time and the adversity that you or your family member is facing.

Yes. Give Liberty Healthcare a call to do a change of Provider request if you already have services. If you do not currently have services or if you personally have COVID you will have to inform everyone that you speak with to see the current requirements or qualifications needed to move forward. If you're services has been terminated you may have to reach out to your doctor's office for a new referral form to be aren't to Liberty for processing. You will be required to have general information at the time of the call. I hope this helped.
