An original or certified birth certificate, social security card or something official with his SS#, proof of residency such as a piece of mail or magazine with his name on it & when we went last week we were missing a photo I'd so she instructed me to log into "Skyward" which is the school management system, go to my child's information page that has their photo, address & basic info & to print that age & bring it in. For farm permit your child will still need to pass the "written" test which is multiple questions with multiple choice answers. Hope this helps!

It sounds like you've already taken the written exam? If so, you won't have to take it again. If you're applying for your permanent license, you'll only need to take the driving test. If this is for a renewal, you only take an eye exam. Hope this helps
