only giving two stars because nate is extremely helpful and actually talks to you with no attitude. I know working customer service isn't the greatest but there is this one lady Cheryl or Sheryl really needs to check herself. She was rude and condescending, I had a simple question that I know she couldve assisted with but her lazy butt just didnt want to deal with anything. She kept saying that it wasnt her department so not her problem, asked her to transfer me to that department..she replies "call back tmo"(knowing theyre closed tmo) asked her to transfer me to somewhere that could help... her response, " no you should call back tomorrow or wait til Nate is in" Seems like im not the only one that has an issue with this cheryl lady.

No. The forms are now being given out through the federal gov. Not state. I believe the form numbers are now MCSA-5875 and MCSA 5876. Website: fmcsa.dot.gov for forms from California

Not from this branch. You would have to go to a regular DMV Field Office with the appropriate forms, documents, and fees to obtain an I.D. card. I would recommend either calling or going online to set up an appointment to save yourself some time. Either that or show up early to try to get in early.
