We cannot, however, answer questions or assist in completing the forms without an appointment. Hi Ara, Unfortunately, our regional offices are currently closed. You may submit your form by mail to: CalPERS Benefit Services Division P.O. Box 942716 Sacramento, CA 94229-2716 We recommend certifying your mail to us so that you're able to receive verification that your form was delivered successfully. Once your power of attorney document has been processed, a confirmation letter will be sent to the address we have on record. The letter will indicate any limitations, depending on the type of power of attorney you have executed. Hello Angelo, Medicare is a federal health insurance program regulated by the Centers for Medicare and Medicaid Services (CMS). As a member currently enrolled in a CalPERS Medicare health benefits plan, you don't need to provide CalPERS with your new card or MBI number. We will obtain your new Medicare Beneficiary Identifier (MBI) number directly from the CMS and update your account. No action is required from you for the transition to the new MBI number.
