I became disabled and since I was a senior, just downloaded the application online, filled it out, then made a copy for myself and one for my doctor before sending it in. They made an appointment for me within a month of sending it in. Met and interviewed with the eligibility worker. Within three weeks, received my paratransit card and welcome package. If you are disabled, cannot take MUNI, then apply.

I don't recommend someone use this company, they don't responsible to costumer reserving schedules, the paratransit van always delay time, sometime no show up, and needs to call many times, but still delay around 1 to 2 hours, or no van show up to pick you up, beside that representative is very rough while you making the appointment. Today, my mom returned time at 1:00pm from dialysis center, no one pick her up, then I called 4 time, she waited until 3:00pm, still no van show, finally, she needed to walk a long distance to the bus stop to take bus. Very bad service.

Very rude driver works for SF Paratransit. Yesterday, 10/27/14 at 3:45pm, I was changing lanes and this dark skinned overweight SF Paratransit driver (with License plate 1366074) starts honking at me, like I cut her off. Maybe she wasn't paying attention but I signaled for at least 5 secs and waited for a long gap to merge into her lane. I called their company to complain and the reception told me that their get a lot of complaints. The reception forwarded me to the complaint department but I couldn't leave a message because it hung up on me. I would not trust my elder friends and family here!

Ina, both previous answers are correct. I am the taxi monitor for paratransit. The number on the back of your card can be a bit tricky so you should call it from a land line in a quiet location. Taxi drivers can tell you your balance up to 75.00. Finance, or customer service, will be able to tell you the exact balance as well as the amount in your reserve, if any. Also, we now accept credit or debit card payments over the phone!
Trudy, yes you'll need to bring your friends SFP debit card to make the payment...there's no online payment
Yes. We actually go within 3/4 of a mile from the last muni stop which includes the daily city bart station. Then, we went a bit further and included westlake shopping center (parking lot).
