I recently had a short term rehab stay at this facility. It is kept pretty clean and they are working hard to catch up on maintenance issues from the previous owners. I looks older but inside is pretty nice. They provided transportation to dialysis while I was still on Medicare. They could use a day room with chairs and a TV for residents so you don't have to always sit in wheelchair or bed. The rooms are pretty tight so little room for comfortable chair in the room. The rehab is excellent. I went from being unable to move in the bed independently to walking some distance with a walker, dressing and washing myself, and toileting myself mostly. Tina with OT was instrumental in helping me rehab. She was encouraging and skillful in getting me to make efforts. John in PT was kind but very encouraging to make the effort to progress. He is skillful and consistent. The food is really horrible. It is not appropriate for diabetics or renal patients. The taste and textures are unappetizing. They respond positively when you complain but no real changes were made during my stay. The day time staff was mostly kind and helpful. The night staff was unkind and made me feel like I was bothering them. Many expressed the thoughts I didn't really need the help I was asking for. There were exceptions to this and once I could go to the bathroom on my own I didn't care so much. But my heart broke for others who could not get call lights answered in a timely fashion. Early in my stay, I was placed on a laxative and then had horrible diarrhea for days which prevented me or made me miserable at dialysis a few times. I would have appreciated some critical thinking about what was causing my problem. ALL IN ALL they did their job and rehabbed me. I was a horrible complaining patient but they still did their job. Being in rehab is not easy. I would recommend this place for people who need nursing home care and rehab. But be vigilant as with all facilities that proper care is being delivered.

As the director of this beautiful home for 5 years. I have the most amazing team to work with every day. For the past 1 1/2 years we have endured this terrible virus COVID. Our staff put their lives on the line everyday and they do it without hesitation. So proud to be a part of this amazing team!

I recently had a short stay at this facility and would like to comment on the overall care I received. I found the food to my liking, they just had a change in kitchen stuff, which added to the plus side of cooking and service. The building is clean and has a staff on duty 24 hours a day, and nurses on call. The therapy room is staffed with their own equipment and personnel. As I reviewed the service, I was provided with during my stay, a five star recommendation comes to mind.

The staff at Signature cover up their indiscretions and care by failing to disclose changes in care in the patients chart. Therapists do not record issues the patient is having with pain or problems they may be having that may influence their progress but would rather release them from care rather than work with them leaving them to fend for themselves helplessly with fractured backs. This same patient has never been taken to a doctor before these same Physical Therapists determined she should be released from the facility. Since when do Therapists have more authority than physicians. Night nurses have been caught substituting medications without doctors authorizations which is illegal and had to be brought to the administrations attention by a family member. Communication between the facility and family is poor leaving elderly patients fending for themselves at times. Very disconcerting. I would not recommend this establishment.
