If there is a lawyer assigned to the probate case, contact them. Let them know what's going on and see if they can request a hearing. Alternatively: Go to the probate office on the third floor, speak to whoever is at the estates window and ask if you can request a hearing.

I am guardian for my younger sister. My Dad is the conservator. He is 85 y/o now. I would like to relieve him of the conservatorship duties and transfer them to my older sister. What form do I fill out?

The first thing you should do is go to the probate office and request a hearing. Lawyers are not necessary, though you need to be patient and go through the proper channels. There may also be legal organizations that will assist in the process. In any case, don't be afraid to just go to the courthouse and ask for a hearing. As long as you act like a civilized person, you'll be able to get something done. The courtroom process is remarkably simple in probate. You'll sign in at the podium, sit down, and wait for someone to come escort you into the courtroom. The judge will ask you what you're here for, and you will tell them. You will bring any evidence you have and offer to show it. Any evidence you offer will be kept by the court, so print out anything you have on your cell phone and copy videos to thumb drives. Please note that, as a part of your request for hearing, the other parties on the case will be served notice of the hearing. This means that they will be free to show up.
