You can obtain it directly from the Health and human services dept. It took less than 5 minutes to fill it out there. Bring your current drivers license or passport. I was told that you can download the application online by going to their website for these who no longer live in the area.

Depends on what time you go. Try to go before noon. I went in around 10am and only had 1 person in front of me. In and out within 10 minutes. As I was leaving there was already a line waiting. The day prior I went after 12pm and there was a long wait.
