I tried countless times, each one denied. I even tried editing ADT as a business that has gone out of business, as ADT American Fork is currently for sale. Google Maps seriously has flaws if someone can highjack a legal site and there appears no way to correct it.
No. Just the hike to the cave involves a steep but paved trail of 1.5 miles with an elevation gain of some 1,000 feet. Strollers and wheeled vehicles of any kind are not permitted on this very steep trail.
Nope. It wouldn't change the experience in the cave. Hiking at night can be done in many areas around the canyon though.

Yes, it's a hike but totally worth it.
It depends on your disability. There is no wheeled vehicles allowed on the very steep pathway to the caves. So, no wheelchairs or strollers. I hiked using forearm crutches, so the caves were accessible in this manner. But i have to admit it was a tough go. If in a similar situation, make sure you ask for added bonus time for walking. I missed my scheduled departure time, but because i was solo, the kind guide in the next group allowed me access on her tour. Service dogs (only) are allowed if that is necessary for your disability. BTW, I didn't have to crawl through any of the tour. The greatest impact on my regular tour, being physically disabled, was the intense hike to the start of the cave tour.
We made the hike with my 76 year old grandmother. She was breathing hard, and we had to stop in the shade often, but she made it and was happy. Along the way we met some men coming down that were saying "I'm 81, so I know you can do it!". At the same time, EMTs were rushing up the mountains with a gurney to bring down a woman that looked to be in her late 20s, hooked to an IV. Stay hydrated, take it slow, stick to shade. They suggest leaving yourself 2 hours to hike before your cave appointment and you will need it. Very worth it! The caves are about 42°, which is immensely refreshing after the hike. Enjoy!
Yes, somehow a company that has gone out of business has taken over the NPS site. I have tried to return it to a park site, but Google Maps has just now denied my plea. This is crazy and difficult to change. I know of two historical landmarks that are listed as bus stations, and no matter what I do to try and correct wrong information, Google either has no desire or a way to correct obvious mistakes. I am finding Google Map's lack of response in necessary corrections frustrating and just plain wrong.
