The card has a website that you can go to to register. Go there and follow the prompts. This is best. Leaving it with the server, does not guarantee the same results.
Yes, there is a physical rewards card which you can register on the silver diner app or on their website I believe. Look on the back of your card and check for instructions

Or you can do it on your phone with the Silver Diner app. If that doesn't work, you can always ask you server during you next visit. But, i believe there is no "physical" card.
