Probably make a appointment to your local mva office. If your drivers license is still good don't worry. I rather visit them in person and get paperwork there.
Normally I'd say give them a call. If you have a provisional license, and you've served your time, should only need proof of address and citizenship. If you have nothing, you'll need to start the learner's permit process. I believe that is addressed in a different question.
