I received forms from SSA and my attorneys that they require a physician to fill out. My primary physician informed me she cannot take care of these items, rules prevent this. Who at JPS do I need to talk to?

I would get with JPSH administration ask why your forms can't be filled out by competent physicians. Maybe they are residents. But they have someone they refer to for things. They are there to learn the ins and outs of healthcare and filling out forms is a part of it. Good luck with your situation.

Dr. told me she cannot. So did cardiologist. Orders from top it seems. Social worker supposed to get back to me.
