You cannot obtain a birth certificate through the Social Security Administration. A birth certificate is considered to be a vital record and must be certified by the state in which you were born in order to be considered legally acceptable. You can order your certified birth certificate copy online through the Illinois Department of Public Health website (dph.illinois.gov). You will be required to provide them with a valid, up-to-date, government-issued photo ID., complete their Application for Search of Birth Record Files form and pay a nominal fee. This copy will include all of the information collected at the time of birth. If you prefer, you may also receive your copy by mail or fax.
Visit the Social Security website to schedule an in-person appointment or find what you need online.
