I have a Dr. Paper for renewal of my Handicap Placard, but I haven't gotten to the DMV in a few months for various reasons. Is there a limit on on how old the form can be since the date the Dr. signed it?how old

No. you are fine bringing in original paperwork. It will be dated acording to what the Dr. dated as duration of time for placard. Hope this helps.

You can still use the Disabled Placard Application that was approved by your doctor. You could have mailed the form to the NV DMV Headquarters up in Carson City instead, their address is on the top right hand corner of the form, and you will receive your placards in the mail within 5-7 business days. Just mail the form that way you won't have to go to your local DMV office to get your placards.
