I found Mercy One does have a Plastic surgery clinic 411 Laurel St. Des Moines Iowa 515 643 5455. I have found no free ride, try contacting nearest training centre see if resident cosmetic surgery program with discount.
I don't think any state run hospitals have plastic surgeons. But they can recommend.
